Frequently Asked Questions

What are the five health checklist types available?
Daily Wellness Screen (pre-entry symptom check), Annual Health Review (yearly records check), Sports & PE Clearance (pre-activity screening), Return to School (post-illness clearance), and Mental Wellbeing Check (frequency-based wellbeing screening).
What is a Critical item and how does it affect the result?
Critical items represent the most serious health and safety requirements. A single 'No' answer on any Critical item immediately sets the result to Not Cleared — regardless of how all other items were answered. Non-critical 'No' answers result in Action Required.
How does the Mental Wellbeing Check differ from the other checklists?
The Mental Wellbeing Check uses a frequency scale (Often / Sometimes / Rarely) instead of Yes/No. Any 'Rarely' answer or three or more 'Sometimes' answers triggers a Support Recommended result. One or two 'Sometimes' answers produce a Monitor & Support result.
Does this tool store any student health data?
No. All data is processed entirely in your browser and is never sent to any server. No student information is stored or transmitted. The PDF report is generated locally on your device.
Can I use this tool for multiple students in one session?
Yes. After completing a check for one student, click Reset All to clear all answers, update the student details, and complete the checklist for the next student. Download a PDF for each student before resetting.
